Steps to becoming a patient with us:

You may also contact us via email on contact@halomedicalclinic.ca and we would call you back, remember to include your contact telephone number in the email!

When you have a confirmed new patient appointment kindly click on the "New Patient Questionnaire" button at the top of this page.

The form is fillable online so you may complete the form before you print it out. You are also welcome to download and print out the questionnaire and complete by hand.

Please complete the form with as many details as possible to help us have a good overview of your health and so we can maintain accurate records for you. Kindly note that the form is not used to determine whether you will be accepted as a patient or not!

When coming in for your new patient appointment, bring your completed form with you.

If you do not have access to complete and print out the form beforehand, not to worry, our helpful and professional staff will provide you a form to complete when you come for your new patient appointment.
